This form was created as an easy way to come forward (anonymously or not) and report any misconduct within the UK Melee/Smash community.
If anyone within the community has made you or another member of the community uncomfortable with their actions, through harassment, abuse or any other inappropriate behavior, no matter how significant you believe it to be, please come forward through this form if you would rather not do so publicly.

This form does not require login. We do not store your data. Your message is relayed to the player safety panel. Only the PSP will have access to see the information you've written on this form. We do not share this information with anyone else.
